Cancer is one of the most severe of the potential complications following kidney transplantation. Post-transplant malignancy poses the dilema for a nephrologist as to whether immunosuppression should be reduced, weighing the risk of rejection against cancer outcome. This Review by Cheung et al. discusses the bidirectional relationship between CKD and cancer, and provides guidance for the management of cancer in kidney transplant recipients.
