ERA-EDTA Funded Clinical Trial Awarded to EURECA-m: ready to start
Volume overload is a leading risk factor for death and cardiovascular events in end stage renal disease patients maintained on chronic dialysis, particularly in those with myocardial ischemia and heart failure which represent a substantial fraction of this population. Early identification of volume overload may prevent cardiovascular sequel in these patients but clinical signs of volume expansion are unsatisfactory to reliably identify patients at risk and to monitor them over time. On the other hand, however reliable, standard techniques for measuring extracellular or circulating (blood) volume do not convey information on fundamental heart function parameters that determine the individual hemodynamic tolerance to volume excess and the response to ultrafiltration,i.e. left ventricular (LV) filling pressure and LV function.

Extra-vascular lung water is critically dependent on these parameters and represents a proxy of both, circulating volume and LV filling pressure and function, and may therefore be a better criterion to identify patients at a higher risk of volume-dependent adverse clinical outcomes and to monitor the effect of therapy aimed at preventing these outcomes. Recently a fast (< 5 min.), easy to learn, simple and non-expensive technique which measures extra-vascular lung water by using standard ultrasound (US) machines has been validated in dialysis patients. Whether systematic measurement of lung water by this technique may translate into better clinical outcomes in ESRD patients has never been tested.
On behalf  of the EURECA-m Board  it is a great pleasure for us to inform you that in March the Council   of the ERA EDTA  approved   the  final budget of LUST,  a   randomized  trial  by EURECA-m  which was  selected  by the  Scientific Advisory Board of the ERA EDTA for  funding among  those  submitted to the same  society in 2011.
The aim of this randomized clinical trial is that of testing a treatment policy guided by extra-vascular lung water measurements by ultrasound  to prevent death, decompensated heart failure and myocardial infarction as well as progression of LVH and LV dysfunction and hospitalization in high risk dialysis patients with myocardial ischemia (a history of myocardial infarction with or without ST elevation or unstable angina, acute coronary syndrome documented by ECG recordings and cardiac troponins or stable angina pectoris with documented coronary artery disease by prior coronary angiography or ECG) or overt heart failure (NYHA class III-IV).

Centers that  will participate into the trial  will receive a  hand-held ultrasound machine and  specific training both in  a  brief  Skype-based  course  and  in a meeting to be held at the Center of Clinical Physiology, CNR, Pisa Italy. EURECA-m members interested in participating into this trial should have   solid links with cardiologists and the possibility of performing detailed echocardiographic studies at pre-specified time in the trial. Interested members should submit:
a)  Their CV and the CV of the cardiologist they may involve into the study. 
b) Characteristics of their Renal Unit  (Academic/non academic; number of patients on hemodialysis; Easiness of arranging Echocardiographic studies; Prevalence of high risk patients, as detailed above).

Many young investigators, particularly from developing countries leave their home country to train at institutions in developed regions and gain international experience in highly reputable research centers, several of them supported by ISN or ERA-EDTA. After time abroad, most of them have difficulties finding their place and acquiring national and international visibility to start their academic careers. To support them, the ISN Young Nephrologists Committee leaded by Roberto Pecoits-Filho has launched a new educational activity. The first meeting of this kind will kick off on September 5, 2012 during the Brazilian Congress of Nephrology and will focus on vascular calcification. It is supported by ISN, the Brazilian Society of Nephrology, the University of Picardie Jules
Massy
Verne and the French Institute National de la Santé et Recherche Médicale (INSERM) and is being sponsored by AMGEN. The congress has an international scope, with an international scientific committee, composed of high level specialists in this area, and well recognized institutions such as University of Washington (Seattle, USA) and INSERM (U-1088, Amiens,
France) while up to date vascular calcification and  uremic toxicity topics will be with active participation of young Brazilian colleagues. Young and seniors researchers from around the word are welcome to participate and to joint us.
