Summary1

Systemic lupus erythematosus (SLE) is a multisystem autoimmune disease of unknown cause in which overt renal disease is the most common severe manifestation occurring in at least one-third of patients. Lupus nephritis (LN) is a major risk factor for overall morbidity and mortality in SLE. Despite potent anti-inflammatory and immunosuppressive therapies, it still progresses to chronic kidney disease (CKD) or end-stage renal disease (ESRD) in many patients, therefore novel therapeutic approaches are essentially needed.

Can antidiabetic drugs prevent chronic kidney disease progression?

Summary2

Hyperglycaemia and diabetes mellitus induce several hemodynamic changes that lead to glomerular hypertension and supranormal glomerular filtration rate. Under chronic hyperglycaemic conditions, increased proximal sodium–glucose co-transporter 2 (SGLT-2)-mediated reabsorption of sodium (Na+) and glucose exposes the macula densa to lowered Na+ concentrations. This impairment likely leads to inadequate renal arteriole tone, causing an afferent vasodilatory response and increased renal perfusion.
