The European Renal Nutrition (ERN) Working Group: research and education on Protein-Energy Wasting management

Since its creation (in 2015), the European Renal Nutrition (ERN) Working Group aims at conducting research on the impact of lifestyle, diet, nutrient homeostasis and Protein-Energy Wasting (PEW) management on Chronic Kidney Disease (CKD) outcomes. ERN, in collaboration with the CKD-MBD Working Group, recently published an important paper in the Journal of Cachexia, Sarcopenia and Muscle: Vitamin D, a modulator of musculoskeletal health in chronic kidney disease (Pablo Molina et al.).

ERN is also committed to the education of nephrologists as well as of nutritionists, endocrinologists and dieticians. An educational meeting will be organised in Belek (Antalya, Turkey) on October 18-19, 2017 and will focus on ‘Protein energy wasting and kidney disease: therapeutic challenges and opportunities’. Speakers and attendees are expected to discuss nutritional strategies to slow progression, as well as address nutritional management, in acute and chronic kidney disease and on how nutritional approach can change in the elderly or in diabetic patients.

A high K+ gradient is related with a greater risk of all-cause hospitalization

A huge gradient between serum- and dialysate-potassium (K+) at the beginning of a hemodialysis-session causes a rapid lowering of serum K+ and could be associated with a greater risk of adverse events. In this retrospective study the association of the clinical outcomes and the K+ gradient (multiple observations per patient, pre-dialysis serum K+ minus dialysate K+ concentration) was assessed in 830.741 dialytic sessions.

Results of the study published in NDT showed an association between higher K+ gradients and fitter patients (younger age, more often fistula use, less comorbidities, better nutritional status). After adjusting for patient differences, there was a dose–response relationship between higher K+ gradient and greater risks of all-cause hospitalization and emergency department visit. But there were no associations with mortality, possibly due to a low number of events.

Finally, an alternative for CKD patients with resistant hypertension? Renal denervation using carbon dioxide renal angiography

Renal sympathetic denervation is a treatment option for patients with resistant hypertension, who have a blood pressure of more than 140/90 mmHg, although they take three or more antihypertensive agents at optimum doses. Especially CKD patients with resistant hypertension could benefit from renal sympathetic denervation, because they usually have a high sympathetic drive. Renal denervation however is often not performed in these patients due to a risk of contrast-induced nephropathy. In most renal denervation studies CKD patients have been excluded.

In new pilot study published in CKJ eleven patients with moderate to severe CKD (eGFR 15–44 ml/min/1,73m2) underwent renal denervation using carbon dioxide renal angiography. The treatment was highly effective; the median blood pressure was lowered to about -14 mm Hg after six months. Using CO2 angiography seems to be a promising alternative that can be used to perform RSDN in patients with significant renal impairment.
